This checklist has been constructed to help explore further whether you may be affected by a clinical depression. For further information, please contact the Mental Health Center of North Iowa or another mental health professional.

The Diagnostic and Statistical Manual for Mental Disorders—Fourth Edition identifies not one but a number of conditions that may be defined as a type of depression or that carry depressive features. The checklist below has been developed not for diagnosis but for better understanding of how depression can affect personal functioning.

 

Persistent change in mood

Withdrawal from usual family and friends

Change in grooming or appearance

Change in sleep habits

Change in energy or activity level

Change in appetite

Tunnel thinking

Psychosomatic complaints

Feeling hopeless

Feeling empty

Feeling worthless

Feeling helpless

Unexplained impulsive behavior

Loss in ability to experience pleasure from daily life

Problems with memory

Problems in judgment

Rigidity in thinking

Problems with concentration
